Prayer Quilt Request

INSTRUCTIONS FOR REQUESTING A PRAYER QUILT

Please Read Before Completing Request Form

1. Ask recipient if he/she would like to receive a prayer quilt. This is necessary because their personal information will be seen by many.

2. Fill out request form below. Your request will be sent to the church office and shared with the Prayers & Squares group.

3. We meet Tuesday to fulfill requests. The quilt will be on the altar the following Sunday. Requests received after Tuesday of each week will be on the altar the next Sunday.

4. The quilt may be picked up after Tuesday. Please call first -(252-752-1273) to be sure that it is there and arrange a time. Salem UMC does not mail prayer quilts.
